INSURANCE

I am an in-network provider with Pacific Source, Aetna, Regence Blue Cross Blue Shield, and Moda. I can also provide a superbill that can be submitted to most insurance plans for out-of-network reimbursement.

I verify benefits and bill insurance on behalf of my clients; however, I recommend that you contact your insurance company prior to our first appointment to discuss your specific benefits, including details about your co-pay and deductible.

Some questions you may want to ask your insurance company include:

• Do I have coverage for outpatient mental health treatment?

• Does my health plan cover out-of-network providers?

• Is pre-authorization required?

• Have I met my deductible?

• Is there a maximum number of therapy sessions allowed under my current coverage?

RATES

My professional rates range from $175-225 per session. However, rates can vary depending on a number of factors including insurance coverage and type of appointment. Please contact me directly to discuss the details specific to your needs. I am able to accept payment via cash and credit or debit card at the beginning of each session.

CANCELLATION POLICY

We will hold our sessions regularly at a mutually agreed upon time. Once an appointment is scheduled, it is important that you provide at least 24 hours advance notice of cancellation. If you do not show up to the scheduled appointment or do not give 24 hours advance notice of cancellation, you will be charged $100. Please note that insurance companies will not reimburse for missed or cancelled appointments.
